2. Counter-Notification

If you believe that content you posted was removed by mistake or misidentification, you may submit a counter-notification including:

Your name, address, phone number, and email.
Identification of the removed content and its previous URL.
A statement under penalty of perjury that you believe the material was removed due to mistake or misidentification.
A statement consenting to the jurisdiction of your local court.
Your physical or electronic signature.
